His right leg and both his hands would have to be amputated for him to survive. Within a few days, his right leg was amputated at above knee level. He could stand and walk with the help of an artificial right leg. However, the artificial hands were not of much use. The artificial hands were not fully functional and he found them to be useless for most activities. There were skin grafts over both the amputation stumps and the underlying structures were scarred and contracted. Hence the recipient preparation was very challenging. Also, the donor's hands had to be dissected and prepared in a different manner so as to match the recipient's requirements. The coordination of the donor and the recipient teams was very crucial, as the donor hands had to travel from Surat to Mumbai, and the recipient had to rush from Ujjain to Mumbai. The surgery lasted for more than 12 hours. The patient had an uneventful recovery in intensive care. He was discharged on the 2<sup>nd</sup> March. The patient was mobilized out of bed within 4-5 days of the surgery and passive physiotherapy started. He is likely to gain reasonable hand function in the next 6-9 months. The case highlights the importance of prompt and comprehensive interventions in pediatric oncology, demonstrating positive outcomes even in aggressive tumor scenarios.</p><p>The multidisciplinary approach, including neoadjuvant chemotherapy, wide local excision, and reconstructive surgery played a crucial role in achieving a tumour cure.</p><div>The six-year-old child’s journey unfolded in Uzbekistan in December 2022, where she presented with complaints of a neck lump in the occipital region. The child subsequently underwent surgery to remove the lump. Unfortunately, fate had it otherwise as the neck mass returned in the months following surgery and again, she had to undergo the knife for excision of the lump. Post second surgery, the patient’s family moved to India to seek further management as she experienced tumor recurrence with rapid tumor growth this time around.</div><p>“The little one presented at Aakash Healthcare Super Specialty Hospital, Dwarka with a huge mass in her neck. The medical team at Aakash immediately sprang into action - a contrast enhanced MR imaging (ceMR) of the neck revealed a large, well-defined lobulated mass lesion (9.4 x 7.1 x 12 cm) in the right posterior neck space. A biopsy indicated a round cell tumor with a Ki-67 index of 90% and was suggestive of PNET/Ewing sarcoma, Dr. Parveen Jain, Senior Consultant & Head of Department, Oncology, Aakash Healthcare Super Specialty Hospital, Dwarka, said,</p><p>The case was discussed in a Multidisciplinary Tumor Board (MSC), leading to the decision to proceed neoadjuvant chemotherapy (NACT) followed by local therapy.</p><p>Shedding light on the further management of the case, Dr. Arun Kumar Giri, Director, Department of Surgical Oncology at Aakash Healthcare Super Specialty Hospital, Dwarka, said, “The patient underwent three chemotherapy sessions that led to significant tumor shrinkage followed by surgery. Massive screening programs will be rolled out by over 44,000 dedicated pediatricians from various clinics and hospitals spread across the country under the program "Anemia Ki Baat, Community Ke Saath,” to screen every child for anemia, ensuring early detection and intervention at no cost.</p><p>“We anticipate reaching over a million people through various media platforms and onsite activities through this program. The program will run for two years and will cover other topics including Screen Time, Obesity, Autism, Down Syndrome, Thalassemia, and many more every month. The increasing prevalence of time-related issues such as obesity, autism, Down syndrome, and thalassemia in children has become a pressing concern. The fast-paced nature of modern life often leads to unhealthy eating habits and sedentary lifestyles, contributing to the rise in childhood obesity. margin-right: 1em;"><img border="0" data-original-height="400" data-original-width="600" height="426" src="https://blogger.googleusercontent.com/img/b/R29vZ2xl/AVvXsEjjqAJ0urOZhDC4QRjSfIOnXu42K0-sBxFEmpUxvEszvFCofuAVJMDIrlKe4vK9F-yf7RUVJRPMNtrLLhbwlRlrfG8Jrv3MFBTrUcN1WhWcU5Z3OCzG002GHN6XWY53Xj60_V71PrMruLbNEaU_cXxT4oeI3M9pmhAC4R6p9_yo6cChL3x0bcaAvVU1mlo/w640-h426/healthy-liquid-beautiful-brunette-woman-600nw-1891473649.webp" width="640" /></a></div><br />– विनोद कुमार<p></p><p>क्या आप भी शरीर में तरलता (हाइड्रेशन) बनाए रखने के लिए बहुत ज्यादा पानी पीते हैं? यदि हां, तो सावधान हो जाएं और तुरंत ऐसा करना बंद करें!</p><p>क्या आपने कभी यह सोचा है कि पानी को आमतौर पर जीवन का पर्याय क्यों बताया जाता है? ‘जल ही जीवन’, ऐसा इसलिए कहा जाता है क्योंकि इसमें इंसान के अस्तित्व के लिए जरूरी सबसे महत्वपूर्ण पोषक तत्व मौजूद होते हैं। पानी हमारे शरीर में लगभग पूरी पाचन या चयापचय क्रिया (मेटाबॉलिक प्रोसेस) में शामिल होता है।</p><p>अक्सर हम एक्सपर्ट्स से पूछते हैं कि हमें कितना पानी पीना चाहिए? दरअसल पानी की मात्रा का ऐसा कोई तय पैमाना नहीं है जो सभी के लिए सही हो। फिर भी आमतौर पर एक दिन में लगभग ढाई से तीन लीटर या आठ से 10 ग्लास पानी पीने की सलाह दी जाती है।</p><p>क्यों ओवर हाइड्रेशन भी शरीर और सेहत के लिए अच्छा नहीं है, इससे कई समस्याएं हो सकती हैंः</p><p>o बार-बार पेशाब करना</p><p>o बार-बार सिर में दर्द होना- दरअसल ओवर हाइड्रेशन से दिमाग की कोशिकाओं में सूजन आ जाती है, जिससे सिर पर दबाव बढ़ता है और दर्द होता है।</p><p>o होठों, पैरों या हाथों में सूजन</p><p>o जी मिचलाना, उल्टी होना, शरीर में ऐंठन, लगातार थकान, कोमा जैसी समस्या भी हो सकती है।</p><p>ओवर हाइड्रेशन से कई समस्याएं हो सकती हैं और शरीर की क्रियाप्रणाली पर भी इसका असर पड़ सकता है। ओवर हाइड्रेशन के कुछ दुष्प्रभाव निम्नलिखित हैं।</p><p>वॉटर पॉइजनिंग (जल विषाक्तता)</p><p>वॉटर पॉइजनिंग का मतलब आवश्यकता से अधिक पानी पीने से शरीर में खनिजों की मात्रा असंतुलित होने से है। जैसे हाइपोनाट्रेमिया (सोडियम की कमी होना) आदि। कोशिकाओं में नमक की भूमिका काफी महत्वपूर्ण है और इसके घुल जाने से उनमें सूजन आ सकती है। स्थिति गंभीर होने पर दिमाग की कोशिकाओं में भी जलजमाव हो सकता है जिसका असर पूरे शरीर की क्रियाप्रणाली पर पड़ सकता है।</p><p>उल्टी-दस्त</p><p>शरीर में पानी की कमी (डि-हाइड्रेशन) या अधिकता (ओवर हाइड्रेशन) एक ही सिक्के के दो पहलू हैं और दोनों के कई लक्षण भी समान हैं। जैसे- जी मिचलाना, उल्टी होना, सिर दर्द आदि। इन लक्षणों में शरीर का पानी सोडियम और पोटेशियम जैसे महत्वपूर्ण खनिजों को तरल बनाकर शरीर से बाहर निकाल सकता है। इससे असंतुलन पैदा होता है और उल्टी या दस्त जैसी समस्याएं हो सकती हैं क्योंकि गुर्दे (किडनियां) शरीर में मौजूद पानी की अतिरिक्त मात्रा को सहेजने में सक्षम नहीं होते हैं। ऐसी स्थिति में खनिजों की आपूर्ति ठीक करने की जरूरत होती है।</p><p>हृदय पर अत्यधिक दबाव</p><p>बहुत अधिक पानी पीने से शरीर की कोशिकाओं में मौजूद नमक तरल बन सकता है, कोशिकाओं में पानी भर जाता है और उनका आकार बढ़ जाता है। ऐसा आमतौर पर नहीं होता है लेकिन फिर भी ऐसी स्थिति पैदा हो सकती है। इसका स्वास्थ्य पर बुरा असर पड़ता है। अगर ठीक समय पर ध्यान नहीं दिया जाए तो यह जानलेवा भी हो सकता है। पूरे शरीर की कोशिकाओं में सूजन आने से हृदय पर रक्तसंचार (ब्लड पंपिंग) का दबाव बढ़ता है। वहीं शरीर में सोडियम की कमी होने से ब्लड प्रेशर कम होता है जिसका असर हृदय की रक्तसंचार क्षमता पर पड़ता है।</p><p>पोटेशियम के स्तर में गिरावट</p><p>पोटेशियम एक प्रकार का खनिज पदार्थ है। यह कोशिकाओं के अंदर और बाहर आयनों के विनिमय (आयन एक्सचेंज) में महत्वपूर्ण भूमिका निभाता है। इससे तंत्रिका कोशिकाओं को मांसपेशियों के संकुचन और आराम जैसी प्रक्रियाओं पर नियंत्रण में मदद मिलती है। ओवर-हाइड्रेशन से शरीर में खनिजों का असंतुलन हो जाता है। शरीर में पानी की मात्रा अधिक होने से सोडियम और पोटेशियम जैसे खनिज तरल रूप में शरीर से बाहर निकल जाते हैं जिससे कोशिकाओं में ऐंठन और सूजन जैसी समस्याएं पैदा हो जाती हैं।</p><p>किसी भी चीज की अति शरीर के लिए खराब होती है, चाहे वह पानी हो! सही मात्रा में पियें।</p><p><br /></p>@ हेल्थ स्पेक्ट्रमhttp://www.blogger.com/profile/14084677003109093564noreply@blogger.com0tag:blogger.com,1999:blog-8776175750169855359.post-85894968375483793612024-02-10T19:22:00.001+05:302024-02-10T19:23:37.774+05:30 साहित्य और सिनेमा से मनुष्य जीवन और समाज जीवन अत्यंत प्रभावित - प्रो. सुनील कुलकर्णी<div class="separator" style="clear: both; सकती है। अगले 30 वर्षों में ब्रेन स्ट्रोक के मामलों में 50 प्रतिशत की वृद्धि हो सकती है और इसके कारण 2.3 ट्रिलियन डॉलर का नुकसान हो सकता है। </p><p style="text-align: left;">ब्रेन स्ट्रोके के आधे से अधिक मामले मस्तिष्क में रक्तस्राव के कारण होंगे न कि रक्त के थक्कों के कारण। इस रिपोर्ट के अनुसार ब्रेन स्ट्रोक की रोकथाम में रक्तचाप और धूम्रपान जैसे जीवनशैली कारकों को नियंत्रित करना महत्वपूर्ण है। Cases of pollution-related ailments have spiked in the past few weeks, health experts said.</p><p>Each year, as winter draws near, the air quality in Delhi deteriorates and reaches unsafe levels. The smoke from stubble fires in the neighbouring states of Punjab and Haryana, the emissions from Diwali firecrackers, and Delhi's local pollution sources are all made worse by cooler temperatures and calm winds that keep the toxins from spreading.</p><p>‘‘Although the post-Diwali time in the city this year was relatively clean, the air quality in recent days has reached dangerous levels. According to experts, hospitals' outpatient departments are already feeling the effects. Exposure to high pollution levels worsens existing respiratory illnesses and contributes to new cases of respiratory problems increasing every year exponentially, during October and November. There are a large number of patients returning with intractable cough, shortness of breath and poor energy levels without any fever or other evidence of infection, and their numbers have been mounting’’, said Dr Tribhuvan Gulati, Expert Diabetes, Thyroid, Metabolic Disorders Apollo Spectra Hospital, Delhi.</p><p>With longer exposure to polluted air, Delhi people are not only facing respiratory problems but also triggers heart and neurological problems such as cardiac arrests, strokes and gastric distress.</p><p>Delhi’s total Air Quality Index (AQI) is recorded to be 354. Noida, a city in the national capital area, decreased to an AQI of 406 and remained in the "severe" category, while Gurugram's AQI remained at 346 and was classified as "extremely bad." When the Air Quality Index is between 0 and 100, it is good; when it is between 100 and 200, it is moderate; when it is between 200 and 300, it is poor; when it is between 300 and 400, it is very poor; and when it is between 400 and 500 or higher, it is severe.</p><p>Today, even those who have never had a respiratory disease before present with a strong, uncontrollable cough and difficulty breathing.</p><p>According to Dr Gulati, ‘‘There has been a significant increase in instances of asthma and chronic obstructive pulmonary disease (COPD), a chronic inflammatory lung disease that obstructs airflow. Additionally, he saw a surge in the patient footfalls as he consults almost 2-3 patients with complaints of cough and breathing problems. margin-right: 1em;"><img border="0" data-original-height="844" data-original-width="1240" height="436" src="https://blogger.googleusercontent.com/img/b/R29vZ2xl/AVvXsEjC-p2e1T3JRtl_b60jfO-F8dI3vgmwFDbajEYOQ8k3iucCdKy__Vg6XZeRhbqzE0S4GWum05-vjszz3qAoyA9mUxgeReECY7nNFCQTrF06zYPZaNph5UqTB39CG3rv-Kr9vKiKmu6qOcyFs_rh3-7yO5GFHf8N4L6wg-NqKNEVMNhXDFadoNaSpli2PHo/w640-h436/ek%20pahal.jpeg" width="640" /></a></div><b><p style="text-align: center;"><b><br /></b></p>– विनोद कुमार</b><p></p><p style="text-align: left;"><b>नई दिल्ली‚ 21 जून, 2023:</b> दिल्ली-एनसीआर, बैंगलोर व पुणे में रहने वाले प्रवासी मजदूरों और उनके परिवारों की बेहतरी के लिए ‘एक पहलʺ नामक कार्यक्रम शुरू करने की कल यहां घोषणा की गई। </p><p>यह कार्यक्रम रियल एस्टेट कंसल्टिंग फर्म सीबीआरई साउथ एशिया प्रा.लि. की ओर से बच्चों के अधिकारों के लिए काम करने वाले अग्रणी संगठन ’बाल रक्षा भारत’ के सहयोग से चलाया जाएगा। इसकी घोषणा सीबीआरआई ने कल राष्ट्रीय राजधानी में की। </p><p>सीबीआरई के चेयरमैन और सीईओ (भारत, दक्षिण-पूर्व एशिया, मध्य पूर्व व अफ्रीका) <b>श्री अंशुमन मैगजीन </b>ने इस मौके पर बताया कि सीबीआरई का इरादा है कि सरकार के सहयोग से और बाल रक्षा भारत को बतौर अमलकारी पार्टनर अपने साथ जोड़ कर आगामी वर्षों में लगभग 3 लाख प्रवासी श्रमिकों को उत्तम स्वास्थ्य सेवाएं, औपचारिक शिक्षा मुहैया कराई जाए तथा उनके पोषण में सुधार लाया जाए। सीबीआरई ने इस वित्तीय वर्ष के दौरान सीएसआर कार्यों के लिए लगभग 5 करोड़ रुपए का एक कोष बनाया है, जिसे साल के आखिर तक तकरीबन 15 करोड़ रुपए तक बढ़ाया जाएगा।<span></span></p><a name='more'></a><p></p><p>उन्होंने बताया कि इस साल ’सीबीआरई केयर्स-एक पहल’ सीएसआर कार्यों का लक्ष्य है कि प्रवासी श्रमिक समुदाय के लिए उपचारात्मक और निवारक उपचार के रूप में स्वास्थ्य सेवाएं उपलब्ध कराई जाएं, सरकारी स्वास्थ्य सुविधाओं में स्वास्थ्य प्रणाली को मजबूत बनाना तथा बच्चों, महिलाओं, पुरुषों व किशोरों के पोषण मानकों में वृद्धि सुनिश्चित की जाए। इस पहल के तहत मजदूरों के बच्चों की पढ़ाई-लिखाई में जो कमियां आ रही हैं उन्हें दूर करने की कोशिश भी की जाएगी, ऐसे उपाय किए जाएंगे की उन्हें औपचारिक शिक्षा प्राप्त हो और उन की पढ़ाई निरंतर जारी रहे। </p><p><b>श्री अंशुमान मैगजीन</b> ने बताया कि इस पहल के तहत इन शहरों में अनेक कदम उठाए जाएंगे जैसे की स्वास्थ्य विभाग के सहयोग से नियमित स्वास्थ्य जांच शिविरों का आयोजन। प्रोजेक्ट टीम संबंधित सिविल सर्जन/ मुख्य जिला चिकित्सा अधिकारी (सीडीएमओ) के सहयोग से शहरी झुग्गी बस्तियों में रहने वाले परिवारों की सेहत के लिए स्वास्थ्य शिविर लगाएगी। प्रत्येक स्वास्थ्य शिविर से पहले इन बस्तियों में रहने वाले लोगों को तैयार किया जाएगा, जिसके लिए जमीनी स्तर पर काम करने वाले लोग जैसे आशा और आंगनवाड़ी कार्यकर्ता तथा प्रोजेक्ट स्टाफ उन्हें जागरुक करेंगे। स्वास्थ्य शिविरों में आने वाले लोगों को विभिन्न सरकारी स्वास्थ्य योजनाओं जैसे आयुष्मान भारत, प्रधानमंत्री सुरक्षा बीमा योजना, एबीएचए कार्ड आदि का लाभ उठाने के लिए प्रोत्साहित किया जाएगा। इस कार्यक्रम के जरिए महिलाओं को मेंस्ट्रुअल हाइजीन के बारे में शिक्षित किया जाएगा और रजस्वला महिलाओं को सैनिटरी नैपकीन भी दिए जाएंगे। इन महिलाओं के लिए मातृत्व, नवजात शिशु, बच्चों व किशोर स्वास्थ्य तथा सरकारी योजनाओं व कार्यक्रमों के बारे में जागरुक करने वाली गतिविधियां भी चलाई जाएंगी। इसके अलावा, इस पहल के माध्यम से शिक्षकों और स्कूली बच्चों की क्षमता का निर्माण करके आयुष्मान भारत - स्कूल स्वास्थ्य और कल्याण कार्यक्रम में भी सहयोग किया जाएगा।</p><p>इसके अलावा शिक्षा के पहलू पर काम करने के लिए मल्टी ऐक्टिविटी सेंटर (एमएसी) स्थापित किए जाएंगे। ये सेंटर 6 से 14 वर्ष के उन बच्चों को शिक्षित करने का काम करेंगे जो या तो स्कूल कभी गए ही नहीं या फिर स्कूल की पढ़ाई बीच में ही छोड़ चुके हैं; As mandated by the FSS Act 2006 and the Food Safety and Standards Rules, 2011, FSSAI is responsible for providing training to individuals involved in food businesses, including food business operators, employees, Food Safety Officers, and designated officers. The rates are comparable for high-, middle- and low-income countries, indicating that this is a major health challenge globally.</p><p>Lifetime prevalence was 17.8 per cent in high-income countries and 16.5 per cent in low- and middle-income countries. WHO said.</p><p>"The report reveals an important truth: infertility does not discriminate," said Tedros Adhanom Ghebreyesus, Director-General at WHO.</p><p>"The sheer proportion of people affected show the need to widen access to fertility care and ensure this issue is no longer sidelined in health research and policy, so that safe, effective, and affordable ways to attain parenthood are available for those who seek it," Mr Ghebreyesus said in a statement.</p><div>Infertility is a disease of the male or female reproductive system, defined by the failure to achieve pregnancy after 12 months or more of regular unprotected sexual intercourse. It can cause significant distress, stigma, financial hardship, affecting people's mental and psychosocial well-being.</div><p>Despite the magnitude of the issue, solutions for the prevention, diagnosis and treatment of infertility - including assisted reproductive technology such as in vitro fertilization (IVF) - remain underfunded and inaccessible to many due to high costs, social stigma and limited availability, WHO said.</p><p>"Millions of people face catastrophic healthcare costs after seeking treatment for infertility, making this a major equity issue and all too often, a medical poverty trap for those affected," said Pascale Allotey, Director of Sexual and Reproductive Health and Research at WHO.</p><p>"Better policies and public financing can significantly improve access to treatment and protect poorer households from falling into poverty as a result," Pascale Allotey said.</p><p>While the new report shows convincing evidence of the high global prevalence of infertility, it highlights a persistent lack of data in many countries and some regions.
